Topical Dutasteride is a prescription only medication in the UK which is sometimes used as a hair loss treatment in men and women. It works by inhibiting both type I and type II 5-alpha reductase enzymes, which are responsible for converting testosterone to dihydrotestosterone (DHT). By reducing DHT levels in the scalp, topical Dutasteride can help to prevent hair thinning and promote regrowth. This is particularly useful for treating androgenetic alopecia (male and female pattern hair loss).

Topical Dutasteride vs Finasteride for Hair Loss
Both Dutasteride and Finasteride are effective treatments for androgenetic alopecia, but they differ in their mechanisms and potency. Although both inhibit the 5-alpha reductase enzyme, Dutasteride inhibits both type I and type II 5-alpha reductase vs Finasteride which mainly inhibits type II. This broader inhibition makes Dutasteride more effective at reducing DHT levels in the scalp. As such, there is evidence that it may be more effective for treating hair loss compared with Finasteride. However, this increased efficacy may also come with a higher risk of potential side effects. As such, Finasteride, with its more targeted action, may be better in terms of effectiveness and tolerability. On the other hand, topical Dutasteride may be useful for those who have not responded to other hair loss treatments.

Dutasteride works best in combination with other hair loss compounds and medicines which boost and compliment its effects. The actives we may use in our hair loss treatments in combination with it include:
- Minoxidil: this duo can promote hair regrowth and improve hair density more effectively than using either treatment alone.
- Biotin: encourages keratin production which can strengthen hair.
- Caffeine: stimulates hair follicles and increases blood delivery to the scalp.
- Melatonin: regulates hair growth cycles and protects hair follicles from oxidative stress to increase hair density and length.
- Tretinoin: retinoid that may help increase absorption of hair loss treatment.
- Hydrocortisone: can help reduce scalp irritation caused by hair loss treatments.

Whilst Dutasteride in any form is not licensed to treat hair loss, in the UK it is sometimes used off-label in the treatment of androgenetic alopecia. Important Information
Oral Dutasteride has been licensed for many years as a treatment for benign prostatic hyperplasia. It has an established safety profile and known side effects. Topical Dutasteride works as a potentially powerful DHT blocker for treating certain types of hair loss like androgenic alopecia. However, whilst early data from clinical trials is a promising, these studies are small and often involve microneedling. Larger studies are still needed to full establish the safety and effects of Dutasteride. Whilst not yet FDA-approved, like topical Finasteride, Dutasteride is available off-licence under the care of a medical professional in the UK. It’s also very important to discuss the benefits, risks and contraindications with your dermatologist or relevant healthcare provider.
Safety and Side Effects
Generally, topical Dutasteride hair loss treatment is considered safe when used appropriately. However, some users may experience side effects such as scalp irritation, itching or dryness. In its topical form, there should have lower risks of systemic side effects like sexual dysfunction compared with the oral medication. However, you can not rule out the side effects completely.
Side effects include mood changes, decreased libido, erectile dysfunction and ejaculation disorders, which may persist even after stopping the medication. Other side effects can include breast tenderness, discharge or enlargement (gynaecomastia), testicular pain and dizziness. Dutasteride can lower PSA levels, which might mask the detection of prostate cancer, so you must let your healthcare provider you’re using it when having PSA testing. In rare cases, it can lead to severe allergic reactions, such as swelling of the lips, tongue, or throat, and skin reactions like rash or itching. If you experience side effects, discontinue use and seek medical help.
Contraindications
There are certain conditions and circumstances which may make Dutasteride unsafe including:
- Scalp Conditions: people with scalp conditions like psoriasis or severe dermatitis should avoid using topical Dutasteride unless advised by a healthcare provider.
- Pregnancy and Breastfeeding: Dutasteride is not safe for use during pregnancy or breastfeeding due to potential risks to the baby. Due to its potential to cause birth defects, it should not be handled by anyone who is pregnant or trying to become pregnant.
- Allergies: those with known allergies or hypersensitivity to Dutasteride or any of its components should avoid using products containing this ingredient.
Storage and Shelf Life
Store Dutasteride containing products in a cool, dry place, away from direct sunlight and heat. Always check the expiration date on the product packaging and discard any medication that has changed in appearance or smell.
